Youth Outreach

Youth Outreach Plus
- Youth Outreach Plus provides tier 3, time limited and goal focused case management for youth aged 12-26. Youth Outreach Plus offers a one-on-one approach in working with youth to address their specific needs associated with mental well-being as well as emotional and physical safety.
- Youth Outreach Plus also offers consultation to Youth Outreach workers throughout the province. Consultation builds the capacity of Youth Outreach workers though offering ongoing support and problem solving.
- In consultation with their Youth Outreach Worker, Youth may decide that the supports of a Youth Outreach Plus worker could be helpful. The Youth Outreach worker will then facilitate the connection.
- Youth Outreach Workers can reach out to Youth Outreach Plus workers in their area for consultation as needed.
Youth Outreach Plus is available in CBRM, New Glasgow, Dartmouth/Halifax, Kentville.
PATH
As part of the PATH program, Family Service Nova Scotia offers case management support to youth aged 19-25 who have care experience through a connection with the Department of Community Services. This is accomplished through regular telephone check-ins. Connections to resources are offered upon request or when needs are identified during the check-in.
Youth are referred to Family Service Nova Scotia’s PATH program by Child and Family Well-Being.
Family Service Nova Scotia’s telephone PATH services are available to all youth referred to the program living within or outside of Nova Scotia.
